Attendance Allowance evidence and forms

When claiming Attendance Allowance, you will need to complete a claim form. Supporting evidence can help show the level of care you need.

The claim form

Attendance Allowance is claimed using a form called AA1. You can get this form by calling the Attendance Allowance helpline (the number is on GOV.UK) or by downloading it.

The form asks about your health conditions and how they affect your need for care and supervision. It is important to describe your care needs as they are on your worst days as well as typical days.

Types of supporting evidence

You do not have to provide evidence, but it can help. Useful evidence may include:

  • Letters or reports from your GP or consultant
  • Hospital discharge letters
  • Care plans from a social worker or nurse
  • A statement from a carer or family member describing the help they provide
  • A diary of your care needs over a period of time

Describing care needs on the form

The form asks about day and night care needs separately. Think carefully about all the help you need — including help that a family member or friend provides without being asked. Help with tasks like getting dressed, washing, taking medication, preparing meals, and supervision for safety all count.

Many people underestimate the level of care they receive because it has become routine. Try to describe all the help you need, not just the most obvious tasks.
